Client’s consent for massage procedure

Hereby, I (hereinafter the “Client”) express my voluntary consent to the massage procedure (hereinafter the “Procedure”) provided with the massage device roboSculptor, I am aware of all potential risks and negative consequences and affections might be caused by the Procedure, and I represent and warrant that I have no contraindications for the Procedure.

Hereby, I acknowledge that:

  • there are some contraindications to the Procedure and a medical consultation should be obtained before the Procedure;
  • provision of the Procedure in the presence of contraindications might lead to harm to life and health. In such a case the Company shall not be responsible for causing harm to the Patient's life and health, the responsibility shall be solely on the Patient;
  • the Procedure is not intended (contraindicated) in pregnancy. In case of concealment of the fact of pregnancy by the Patient, the Company is not responsible for any harm to the Patient’s health and life, the responsibility for causing harm to the Patient’s life and health shall be on the Patient;
  • in the case of tattoos on the body, the effectiveness of the Procedure might be significantly reduced;
  • the Procedure is carried out in a suit specifically designed for this massage. Participation in everyday clothing is not allowed;
  • the Procedure is provided in the presence of indications to conduct. If there are no indications for the procedure, the effect of the Procedure may be significantly reduced;
  • the Procedure is aimed at providing AI massage. Provision of the procedure does not guarantee any positive result for the health of the body;
  • provision of the course of Procedures neither guarantees full recovery of the Patient nor guarantees a subsequent relapse;
  • the effect of the Procedure cannot be predetermined and guaranteed in advance.

I acknowledge that unconditional contraindications to the Procedure include the following:

  • Oncology with remission up to 5 years;
  • Acute infectious and viral diseases (including prodromal stage);
  • Hyperthermia;
  • Hemophilia;
  • Chronic diseases in the acute stage;
  • Epilepsy, convulsions;
  • Hepatic, renal, cardiac or respiratory failure in the stage of decompensation;
  • Dermatological pathologies (extensive localization);
  • Inflammatory skin diseases;
  • Inguinal and umbilical hernia;
  • Autoimmune disease with thyroid hyperfunction;
  • Phlebitis, thrombosis, trophic ulcers;
  • Inguinal and umbilical hernia.

With care:

  • Gynecological diseases (benign neoplasms prone to growth: fibroids, cysts, endometriosis) (consult with doctor);
  • Menstruation for the first 3 days;
  • Varicose veins (past stages C4);
  • Pregnancy and lactation (consult with obstetrician-gynecologist).

Hereby, I represent and warrant that:

  • I have duly familiarized myself with the rules of the Procedure;
  • I have no contraindications to the Procedure;
  • I have medical consultation prior to the Procedure to determine if the Procedure can be performed and if there are no contraindications to the Procedure;
  • there are no other circumstances preventing the Procedure, as well as circumstances that may entail a threat of harm to my life and health in connection with the Procedure.

Consent to the processing of personal data

  • FZ Robotic Therapeutics (hereinafter referred to as the "Provider") represents and warrants that the personal data provided by the Client are to be processed subject to the requirements of applicable law, as well as strict compliance with the provisions of the Personal Data Processing Policy (https://www.robo-sculptor.com/privacy-policy), and terms and conditions declared by the Service in this Consent.
  • The Client (You) hereby expressly agrees and confirms their Consent to the processing of their personal data by the Provider. Consent to the processing of personal data shall be deemed confirmed if:
    • You click "register" or "book session" when filling out the appropriate web forms at the roboSculptor interface.
    • You continue to obtain massage treatment with the roboSculptor massage device.
  • The geographical location of the point of connection to the information and telecommunications network "Internet".
  • By confirming the Consent, you grant the Provider the right to process the personal data provided by you in the web forms of the Personal Area.
  • The legal basis for the processing of Personal Data shall be the rules of applicable law governing issues related to the processing of Personal Data, including but not limited to Federal Decree-Law No. 45 of 2021 Regarding the Protection of Personal Data, as well as this Consent to Personal Data Processing.
  • The Provider processes your personal data to the extent necessary and sufficient to achieve one or more or all of the purposes specified below:
    • Providing massage services using the Robosculptor massage device.
  • The Provider may process the following information about the Client, received (collected) in the process of using the Personal Area by the Client within the obtaining by the Client massage treatment:
    • Full Name;
    • Phone number, email address;
    • Height, weight, sex, age;
    • IP address, MAC address, browser name, and version;
    • Cookies.
  • The Provider performs the processing of personal data both with the use of means of automation and without the use of means of automation. The Provider shall not perform cross-border personal data transfer.
  • This consent is given, including the transfer of personal data, to Provider’s partners, other Provider’s affiliates, professional consultants, auditors, and medical professionals.
  • The Provider stops processing personal data in case of achievement of the purposes of processing, receipt of the application of the user/visitor (in cases stipulated by the applicable legislation – by the legal representative of the user/visitor) on withdrawal of consent to processing of personal data by sending the relevant application to support@robo-sculptor.com as well as in cases directly stipulated by the applicable legislation.
  • This Consent to the processing of personal data is valid from the date of its provision until its revocation.
